Essential functions include:

  1. Spending time with each client to assess needs, listen to challenges, coach individuals through their recovery, and refer to concrete services.
  2. Having a caseload of up to 20 people, with the amount of time spent per person depending upon individual need.
  3. Partnering with the client and with community service providers to identify barriers and enact comprehensive plans with appropriate connections to support stability and safety in the community.
  4. Documenting all activities, service plans, and required metrics in the prescribed electronic health record.
  5. Meeting with the potential client pre-discharge, obtaining agreement/consent, and meeting with appropriate collateral contacts at the inpatient setting.
  6. Accompanying each individual on the day of discharge to support and assess primary needs and barriers. Ensuring connection to primary supports, including but not limited to mental health services, SDoH supports as needed, and social/support groups.
  7. Making home visits, engaging in collaborative assessments, meeting with existing supports, introducing client to new supports and giving support and advice to client and caregivers.
  8. Engaging with and supporting primary supports to promote attendance at treatment and SDoH appointments, as well as focusing on long term stability by providing supportive services that will ensure that all long-term services remain in place and that there are no impending avoidable disruptions.
  9. Observing operation of support network, mediating conflicts between client and caregivers, helping modify network as necessary and encouraging the client to take more responsibility.
  10. Most of the interventions should be in-person.
  11. Working a flexible schedule to ensure continuity of care and optimal support services.
  12. Other duties as assigned.

The position requires an Associate's degree in human services, psychology/social work, business management or management or related field and equivalent experience. Bachelor's degree preferred. Experience with behavioral health or social determinants of health (SDoH) systems, including case management or delivery of clinical interventions. Experience with social determinants of health-related programs, including but not limited to housing, employment, food insecurity, and entitlements. Understanding of NH systems of care, with experience working between systems. Comfort with technology and reporting systems, with the understanding that use of the agency's electronic record is compulsory. Knowledge of treatment modalities and interventions such as service coordination, behavior management, independent living skills training, supportive counseling, family education, discharge planning and crisis intervention. Have and maintain a valid state driver's license and personal automobile insurance.
