About the Role

The Nurse Specialist – Triage Lead is a key member of our care team, providing high-quality telephone triage, patient education, and care coordination. In addition to clinical responsibilities, you’ll lead triage operations, train staff, audit workflows, and ensure safe, efficient, patient-centered care. This role is perfect for a nurse who thrives in leadership and collaboration.

What You’ll Do

• Clinical & Triage:
  • Serve as a trusted resource for patients and families with clinical questions
  • Triage acute medical issues using established protocols
  • Communicate test results and care instructions per provider direction
  • Provide patient education and self-management support
  • Document all interactions accurately in the EMR
• Leadership & Team Support:
  • Oversee triage workflows and daily huddles
  • Train and mentor triage nurses
  • Conduct chart audits and ensure compliance with protocols
  • Assist with scheduling, evaluations, and team development
• Care Coordination:
  • Track patients after hospital or ER visits
  • Educate patients on care plans and assist with goal setting
  • Connect patients to internal and external resources for social or financial needs

What We’re Looking For

• Current RN license in Kentucky
• CPR certification
• Education & Experience:
  • Nursing Diploma + 3 years experience OR
  • ADN + 5 years experience OR
  • BSN + 1 year experience OR
  • MSN + 1 year experience
• Strong communication, leadership, and organizational skills
• Passion for patient-centered care and team collaboration
