Title: LMSW Care Manager (Social Worker)
Department: Case Management (MSW)
Type: Travel Contract (13 Weeks)
Schedule: Day Shift | 5x8-Hour (08:00 - 16:30)
Start Date: 09/08/2025

We are seeking a qualified and compassionate Licensed Master Social Worker (LMSW) with at least 2 years of experience in acute hospital settings to join the Case Management team. The role focuses on care coordination, discharge planning , and evaluating patient needs using InterQual/Milliman criteria. This position is ideal for a candidate looking to make a meaningful impact in patient transitions of care while working in a dynamic hospital environment.

• Perform comprehensive case management, including initial evaluations , care coordination , and discharge planning

• Utilize InterQual and/or Milliman guidelines to determine medical necessity and perform utilization reviews

• Collaborate with interdisciplinary teams, patients, and families to ensure timely and safe transitions of care

• Complete continued stay reviews , retrospective reviews , and participate in appeals and denials

• Document thoroughly according to coverage criteria and regulatory standards

• Assess patient needs across medical, social, and community dimensions

• Coordinate referrals to home care, rehab, LTC facilities, and community-based programs

• Work with patients in various hospital units including Emergency Department , Medical-Surgical , and ICU

• Active Washington State LMSW license (required at time of submission)

• Master's degree in Social Work

• Experience with Epic charting system (required)

• Minimum of 2 years' experience in acute hospital case management/discharge planning

• Must not have worked for any Providence-affiliated facility in a perm role within the last 3 months

• Open to candidates with associate clinical licenses or LICSW

• Travel experience preferred , but not required

• Average Daily Census: 18 20

• Must float to similar units as needed

• Master's Degree in Social Work (MSW) Required

• Minimum 2 years in hospital-based case management Required

• Experience with Epic Required

• Community resources and discharge planning knowledge Required

• Active Washington State LMSW or Associate Clinical License Required

• Familiarity with Medicaid, Medicare, and community program eligibility

• Epic Charting

• CMS, HIPAA, ICD-10, OSHA, DRG, and Joint Commission Standards

• Emergency Department, ICU, L&D, NICU Experience

• Experience with patients with psychiatric disorders, HIV/AIDS, dementia, stroke

• Geriatric Depression Scale, Biopsychosocial Assessment

• Experience with vulnerable/high-risk populations

• BCLS Required

• MSW Required

• State Licensed (WA) Required at time of submission

• Travel and local candidates accepted (local candidates paid at travel rate)

• Block scheduling is not available

• Previous Providence employees must have a minimum 3-month break in service to qualify

• Submission must indicate WA license status (Active, In Process, or None)
