Requirements Master's degree in Social Work from an accredited school Preferred: One year experience in Home Health Care Must possess a valid Driver's License, car insurance, and reliable transportation What you'll do: Conduct initial assessments, and help create goal-directed care plans Collaborate with physicians and participate in interdisciplinary team meetings, sharing insights about patients' needs Record all patient and family interactions clearly and on time in patient records Offer reassessments and ongoing counseling for patients and families as needed Engage in crisis intervention and provide compassionate support to patients and their loved ones Actively advocate for patient and family needs Stay connected with community agencies to facilitate smooth referrals Provide thoughtful counseling around end-of-life issues when needed Benefits ~401K Retirement Plan ~ Medical benefits Available ~ Ability to earn PTO ~ Flexible Scheduling ~ Excellent Pay ~ Employee Appreciation program ~ Rewarding Work Environment ~24/7 staffing support
